If you’ve been dealing with sciatica, you already know how debilitating it can be. The sharp, burning pain that shoots down your leg, the numbness, the limping — it affects everything. And if you’re reading this, you’ve probably already tried a handful of things that haven’t fully worked.
I want to explain why that happens, what’s actually going on in your spine, and why chiropractic care addresses sciatica in ways most other treatments simply don’t.
What Sciatica Actually Is
Sciatica refers to pain that travels along the sciatic nerve, which runs from the lower back down through the legs and into the feet. Most people think of it as leg pain, but the problem almost always starts in the lower back.
Here’s what’s happening at the level of the spine. The vertebrae in your lower back can become misaligned, a condition we call vertebral subluxation. When a vertebra is out of position, it can pinch or irritate the nerves that exit the spine in that area. Those are the same nerves that feed into the sciatic nerve. When they’re compressed, you feel it all the way down your leg.
Not everyone with sciatica has lower back pain. Some people feel the pain exclusively in the leg or buttocks. That can be confusing, but the origin is still the same—an irritated nerve root in the lower back. Treating the leg doesn’t fix the problem. Correcting the subluxation in the spine does.
How I Examine a Sciatica Patient
When a sciatica patient comes into my clinic, the examination is hands-on and specific. I have the patient seated and examine the spine from behind, palpating each segment of the lower back to find exactly where the problem is coming from.
Clear signs tell me where the subluxation is located. The area will be tender on one side of the spine. The joint will feel locked up rather than moving freely. That region will typically have associated muscle spasm as well. Together, those findings point to the exact vertebra that needs correction.
That specificity matters. A generic treatment applied to the general area of the lower back is not the same as a precise adjustment delivered to the exact joint that is locked up and compressing the nerve.
Why Most Treatments Don’t Resolve Sciatica
Before patients come to see me, most of them have already been through a familiar cycle. Hot packs, cold packs, pain medication, massage, stretching, foam rolling, and sometimes physical therapy. They may have gotten some relief, but the pain kept coming back.
Those approaches fall short because they target symptoms rather than the source. Medication reduces the pain signal, but the vertebra is still misaligned. Massage loosens the surrounding muscles, but the joint is still locked. Stretching can provide temporary relief, but the subluxation hasn’t moved.
The problem stems from a locked joint in the lower back that is pressing on the nerve. In some cases, the disc at that segment can swell and add to the compression. Correcting it requires a specific force applied to that exact location—a chiropractic adjustment—that unlocks the joint, restores motion, and relieves pressure on the nerve.
A Patient Who Could Barely Walk
One case that stays with me involved a man who came into my clinic limping badly and could barely walk on his own. After examining his spine, I found that his L5 vertebra — the lowest segment of the lumbar spine — was subluxated. The sciatic pain was severe, concentrated in his buttocks and radiating down his leg, and it had taken away his ability to walk normally.
After his first chiropractic adjustment, he felt about 70 percent better. When he returned for his second visit, he walked on his own power. The limp was gone. He was in better spirits. After his second adjustment, he came back for his third visit a changed man. He couldn’t believe it. Two chiropractic adjustments had given him his ability to walk back.
That kind of result isn’t a fluke. It happens when you correct the cause of the problem instead of masking it.
What Your Doctor May Not Have Told You
Here’s something I want every sciatica patient to understand. Sciatica pain can go away on its own for days or even weeks, and then come right back. Most people interpret that temporary relief as the problem resolving. It isn’t.
If the vertebral subluxation hasn’t been corrected, the underlying problem remains. Medication may quiet the pain signal, but the joint is still locked. The nerve is still compressed. And sooner or later — with the wrong movement, a long car ride, or just accumulated daily stress — the pain returns.
Feeling better is not the same as being better.
With chiropractic care, the goal is to correct the cause. When we unlock the joint and restore proper motion to the spine, the nerve decompresses, the body can begin to heal, and the result isn’t just less pain. It’s more mobility. Better function. The ability to go through your day without wondering when the next flare-up is coming.
How Long Does It Take
Sciatica doesn’t develop overnight, and it doesn’t resolve overnight either. For most patients, I recommend starting with three visits per week in the early phase of care. That frequency keeps the momentum going and allows the adjustments to hold between visits.
The good news is that many patients experience significant relief quickly, as early as the first or second visit. But completing the recommended course of care is important. Stopping treatment as soon as the pain subsides is one of the most common reasons sciatica comes back. The joint needs time to stabilize in its corrected position before that improvement becomes lasting.
